Asedau sy'n Seiliedig ar Leoliad

Mae dulliau gweithredu sy’n seiliedig ar asedau yn ffordd o edrych ar yr hyn sydd gan gymuned/poblogaeth, yn hytrach na’r hyn nad oes ganddynt. Mae’r dull hwn yn helpu i werthfawrogi a defnyddio sgiliau, gwybodaeth a perthnasoedd sy’n bodoli eisoes mewn cymuned. Bydd mabwysiadu dull sy’n seiliedig ar asedau o ran cynllunio yn sicrhau bod cymunedau lleol, sefydliadau’r sector cymunedol a gwirfoddol, a’r gwasanaethau statudol yn cydweithio i gynllunio, dylunio, datblygu, darparu a gwerthuso mentrau iechyd a llesiant.

Mae canllawiau NICE ‘Community engagement: improving health and wellbeing and reducing health inequalities’, yn disgrifio:

  • Defnyddio dulliau sy’n seiliedig ar dystiolaeth i ymgysylltu â chymunedau (gweler cydweithrediadau a phartneriaethau a rolau cyfoedion a lleyg).
  • Bod yn glir am ba benderfyniadau y gall pobl mewn cymunedau lleol ddylanwadu arnynt a sut y bydd hyn yn digwydd.
  • Cydnabod, gwerthfawrogi a rhannu gwybodaeth, sgiliau a phrofiadau’r holl bartneriaid, yn enwedig y rheini o’r gymuned leol (ewch i’r adran ar ddysgu a hyfforddi).
  • Gwneud amcanion pob partner ar gyfer ymgysylltiad cymunedol yn glir.
  • Parchu hawliau cymunedau lleol i gymryd rhan cymaint neu cyn lleied ag y gallant neu y maent yn ei ddymuno.
  • Sefydlu a hyrwyddo rhwydweithiau cymdeithasol a chyfnewid gwybodaeth a syniadau (ar faterion fel blaenoriaethau a gwerthoedd diwylliannol gwahanol).

Public Health Wales has developed resources to support the empowerment of individuals and communities who support outcomes one and two of the Primary Care Model for Wales. Actions that motivate greater individual and collective control promote health in its own right. Empowerment also improves social relationships at the level of individuals and the population, and when empowered, individuals engage in the work to improve and develop services, and the provision is better and more likely to meet the needs of service users.

Empowerment is not something that professionals or organisations 'do' for a community, but our role is to create conditions where communities can seize power. The Principles of Community Engagement for Empowerment aim to encourage reflective practice in working with and for communities.

Social prescribing is an umbrella term that describes an individual-centered approach to connecting people to local community assets. The community assets include community groups, interventions and services that could be provided online or face-to-face, as well as buildings, land or even an individual within a community. It can help empower individuals to identify their personal needs, strengths and assets and to connect with their own communities to support their health and well-being.
